Independent Contractor Paperwork

There are several forms you need to complete before you can start working with Access Simplified as an independent contractor (IC or 1099). Please review the brief summaries of each form below. If you do not complete a Form W-9 and give it to us, we will be unable to pay you until we have your W-9 on file.

Access Simplified’s Non-disclosure Agreement (NDA)

All work you do for us remains confidential. All information you learn about us remains confidential.

Direct Deposit (DD)

Access Simplified provides payment via direct deposit.
Please provide your direct deposit information in order to be paid.

Your bank account number

Copy of your voided check OR
Copy of your bank statement
